The VIAL OF LIFE may help save you or a loved ones life. The information may be invaluable to help Emergency Medical Services and the hospital deliver proper emergency care to someone who may not be able to communicate. You or someone else completes the form, places it into an envelope and then places it into your refrigerator. A decal is placed on your door and/or refrigerator door. Emergency Medical Services personnel looks for that information if we respond to your residence for an emergency.

The VIAL OF LIFE kit may be picked up during normal business hours at Station 112 located at 64279 Hwy 3081, Pearl River, LA 70452. If you are unable to pick one up, contact us at 985.256.4705 or email us at contact@pearlriverfire.org and we will get one to you. You may also pring out your own below.
