Print the application, background check and criminal history form.  Fill out and submit to Station 112 at 64279 Highway 3081 Pearl River, LA 70452. If you are unable to open or print a .pdf file, email us at contact@pearlriverfire.org.

 

On the background and criminal history forms, just fill out your basic information on the bottom, other information will be filled out by fire department personnel.

The L4 and W4 Forms are for tax purposes and can be completed upon hiring.

OSE Competitive and Promotional applications are required for the respected type of application and testing but do not need to be submitted at the time of the application, background check and criminal history submittal.
